The per-line checkbox in the procedure (line item) record is the gatekeeper that tells the system to include that line’s HCPCS on the UB form, so the code ends up in field 44.

Other options don’t control this per-line mapping to the UB form. A patient-record flag is too broad and doesn’t guarantee per-line printing. A claim-header toggle wouldn’t dictate how line items map to field 44. And while showing HCPCS on the line item can display the code in item views, it isn’t by itself the mechanism that places the code into field 44 on the UB-04 form without the per-line show-on-UB setting.
